[He sits and offers his hands again, sheepishly.] Doctors don't do this sort of work. They need you. Upstairs.
[John is short and stocky, but stubborn, really. Stubborn about caring about him Which is unusual.]

[He smiles, but then raises his brow at the last part.]
I don't see what your level has to do with it.
[He's a doctor. This is what he does.]

Because you intended to go home, you've had a long shift, you've never noticed me before, and people are usually selfish about their own lives, and their own hours.
Instead you're looking after me.
Now, of course you're a competent doctor, so you prefer busy days, though you care for the injured you prefer that they're more of them. You have a romantic life despite your shifts, though you are unwilling to commit to anyone.
[He shrugs. He's not well-liked for a reason.]

[He's not sure how he feels about being read so completely. Has this man asked around about him or something? He lets the hand go and reaches for the other hand to give it the same treatment.]

[He lets John take his other hand that is just as chapped and sore. He hadn't really paid attention.
John isn't angry. Normally people don't like being read. John didn't seem to mind. What a strange man.]

Now if you'll please answer me who is doing the laundry in my absence, and I'm not fired am I? It cannot be you as you have a shift in the morning, and you cannot do shifts and the laundry. It will go or someone else will have to do it if I am not.
If you ask any of the orderlies to do it, they will ruin my system. It is efficient, the laundry gets done properly and they are sanitized and clean for anyone upstairs to use without fear of infection.

[John gets up and wipes his hands off with a handkerchief in his pocket, then uncaps the eyedrops.]
Tilt your chin.
[He touches Sherlock's chin gently, to lift it up so the man stares up at him so he might administer the eyedrops.]

[He's still not changed out of his soaked trousers and digs his socks out of his shoes, looking a little uncomfortable to sit in the small chair and put them on. It's weird going home so early.
The socks are knitted and have been patched a few times. It's clear he - dresses more than his budget. Wants to be respected and not look vulnerable. Then he realizes what might be implied and shrugs.]

[When Sherlock is ready, John will walk with him outside to the car. It's not really his, it's a loan from the military. If he gets an urgent call, they don't want one of their top doctors sitting on the bus.]

Are you not one for intimacy in front of others, Doctor? You do examine them all day, I don't see why it matters.
[He's either being bold or purposefully scandalous to make the attempt at friendship uncomfortable. He can't figure out why a doctor that just came down to the laundry on a whim decided he deserved care. He'd limp in exhausted some nights after everyone went to bed and no one cared then. He was clean at least, suppose he should be grateful for the distraction.
He doesn't really care about John's driving and ignore it.]

[The road is bumpy and uneven and he doesn't really care, he's more focused on examining his fingers than the conditions.]

[Why was he asking? He's not even sure. It just occurred to him to ask. Maybe it was improper of him.]

Yes of course. [Why does John care about his comfort? It's not like anyone else does. As long as he is not in a drug den or dying his brother does not care about his situation. The fact he had space should be a good thing. He ignores the fact he might be wrong about intimacy.
Why else would John care about someone's comfort.]
