Attacked by a monster. Hit by a nasty spell. Caught in a trap. Used the wrong ingredients in your latest recipeh. However it happened, the side effects are beyond inconvenient and you're all out of remedies.

poison Something nasty is running through your veins, draining your strength a little at a time. You did bring the universal antidote, right?
paralysis Your limbs might as well be dead weights for all the good they're doing you, and it's difficult or impossible to get around without help.
confusion You're delirious, out of touch with reality. It's hard to tell friend from foe, and if you keep this up you could easily hurt yourself or your allies.
silence You've been rendered non-metaphorically speechless. Better get creative if you want to communicate.
blind Whoa, when did it get so dark?
sleep Yawn... so tired all of a sudden... can't keep... eyes open...
polymorph Ever wondered what it's like to be a frog? No? This isn't your lucky day, then.
berserk Something has you seeing red. You want to fight everything, preferably with your bare hands. Even if you're a squishy wizard. Especially if you're a squishy wizard.
charm Whose side are you on, anyway?
mini Being only a few inches tall gives you a whole new perspective on the world. Too bad that perspective is one where all your stuff is much bigger than you.
wildcard Mix and match, make up your own, come up with a smut option, maybe there's an effect I missed, whatever.
