The Darkest Timeline.

[She slowly edges herself off the bed, places the jug back without an offer of more. Her back is turned to him now, a direct statement of what will happen if he says no.]

[Watson has little to do with her business, and once Sherlock's in her grasp, the other woman will be nothing more than an easily-ignored sidenote. She'll leave her be because Moriarty never saw her as a threat to begin with.]

[She's small, but strong and aware of how to use her body, more than capable of carrying most of his weight. Sneaking him out will be easy, the surveillance cameras stuck in a looped feed. Nobody will know he's even missing until the morning, when they're already too far gone to catch.

[He feels somewhat emasculated, being carried by Irene---Moriarty. He thought he was protecting her before, when she had been "traumatized" and changed. Now she was genuinely protecting him, lifting him up. All the strong muscles in his arms and shoulders mean nothing when he can't so much as stand.
He leans against her as she carries his weight.
She still wants him, he tells himself. Even if it isn't the way he wanted it to be, she's still--they're still---she loves him. She has said so.]
This would have been a lot easier for the both of us if we hadn't fallen in love.

[It would have been a lot easier to collect her money and leave without him, to allow her nemesis to waste away back into his heroin addiction. But Moriarty views the risk she wouldn't take for anyone else as a bonus than a liability. After extracting him from the safety of the hospital, Moriarty allows him to rest in the car bound for the airport, buckling him in and pressing a damp cloth to his forehead almost motheringly.
She has luggage already packed for the both of them, mostly to avoid suspicion through customs than for convenience. Moriarty examines their fake passports briefly before tucking them into her bag.]
You'll have plenty of time to rest and recover once we're in Paris.
