There's no need for elaborate backstories for this AU. You go into heat, rut, whatever you'd like to call it. You've always done so since you were of age to do so. It's a socially accepted norm, and people have gotten used to the difficulties that come with the mating seasons. Life goes on.

Isn't this alpha/beta/omega, then? No, not at all. In this meme, there are no gender-variant genitalia unless your character comes prepackaged with those. No giant cocks - again, unless your character is already packing - or self-lubricating assholes or anal wombs, and no knotting. Unless that's your thing. There's also no set in stone roles with regards to dominance and submission, and some people do not even feel the inclination towards either. Still, there are a few similarities, mainly being that the pheromones of others can put someone into heat that was not in it before and the bond between mates. In addition, some humans/human-stand-ins have created packlike dynamics in response, but this is not universal.

In this meme, it's just regular old human...oids feeling the urge for sex. And by "urge," we of course mean all-consuming drive. If characters choose to ignore that urge, it will backfire on them. The more they put it off, the more they will lose their control, becoming more irritable and aggressive, and eventually be nothing but a rutting animal until the need to mate is satisfied. Not all heats call for mating, of course, and most people can just relieve themselves, but when you do mate, you feel the urge to stay by their side and essentially "nest." These desires don't always pan out into offspring, and mating isn't forever unless it's mutually decided upon. However, impregnating your mate is said to be one of the most euphoric experiences a person can have, even if some people only mate because they want to utterly possess someone, as mates are bonded for a while.

Apparently nobody thought to tell Dean about werewolf biology, and all the implications that came with it. Derek figures that Dean should have known anyway, being the big-dicked infamous hunter that he was, but it was reasonable to assume that John Winchester never recorded this kind of behavior in his journal. And really? why would Derek tell Dean about rut in the first place? It wasn't like they were anything special, he assumed. Sure, they've been fucking regularly, hunting together, and pretty much all up in each other's business at this point, but that's just because it was convenient for them. They fit together perfectly when it came to scratching that itch, but Derek never forgot that they're natural enemies, that in the end, getting attached to Dean Winchester would burn him, just like all the others had.

If only common sense would overrule what he wants, how he feels, and how his body was currently reacting...

Normally his rut came fast and hard, a weekend of marathon sex with a willing participant or two, and it was over. Dean would probably enjoy that, and he wouldn't even have to know it was any different from their normal routine. That's how it happened before though, when Derek was mostly alone, when he wasn't falling into bed with Dean Winchester every few nights. This time, it came on slower, more steady, almost so that Derek didn't even notice it. It wasn't the burning arousal in the pit of his stomach, the hormones making him get hard at even the slightest stimulation, worse than a teenager in puberty... No, this time it simmered, came on slowly, and it was entirely focused on Dean.

It was easy enough to ignore at first, just little things like how he stood closer to Dean on hunts, seemed more protective and taking risks so Dean didn't have to, and small touches whenever they were close. It got worse throughout the week, pressing close to Dean, even in public, nuzzling into his neck, scenting him, and even Sam noticed that Derek was doing it intentionally, marking Dean and claiming him so that all others would stay away.

By the weekend it hit a boiling point, the three of them were at some dark, dingy dive bar, when some cute little blonde tried to flirt with Dean, doing just about everything short of offering to suck him off right there at the bar. The moment her fingers brushed Dean's hand, Derek was grabbing her wrist and twisting it away. He moves to Dean and wraps his arms around the hunter, his hand slipping into his jeans to grope him firmly as he nuzzles into Dean's neck again, growling threateningly, with his eyes on the woman the entire time. Sam had to drag them out of the bar the moment that Derek flashed his icy, steel blue eyes at her, baring his fangs in the dim lights...

The drive in back to the motel was silent, but the tension in the air was palpable. Derek could tell that they were waiting for him to explain himself, but all he could hear was the blood rushing in Dean's veins, the rapid heartbeat... And it was a good thing that Sam was driving, because the only explanation Derek is offering is leaning up from the back seat and kissing and nipping at Dean's neck, breathing in the hunter's scent, the spice and gunmetal... And trying to coax him into the back to join him.
