1. Dramatically! Probably in battle or in some kind of noble sacrifice.
   
2. A really stupid accident you don't want to talk about.
   
3. Something drawn out and angsty like some kind of Victorian disease.

5. You don't know! It was pretty quick...

Where are you now?

1. Back on Earth. You came back...somehow, to some degree. A ghost? A zombie? That sort of thing.
   
2. VALHALLA, home of the brave and/or the sneaky people who found a back way in!
   
3. The obligatory darkest reaches of some Stygian abyss. Maybe a lair of demons, maybe hell, etc.
   
4. Limbo. It's really boring.
   
5. It appears to be some kind of carnival and you're not really sure what's going on.

What do you want to do?

1. Fix this death thing and resurrect.
   
2. Take care of some kind of unfinished business.
   
3. Aggressively kill time, maybe because you're in one of the more boring locations like Limbo.
   
4. Take over the afterlife.
   
5. Stop someone else from taking over the afterlife.
